ABSTRACT

ABSTRACT Objective: Recent studies have shown a relationship between adipose tissue and coronary artery disease (CAD). The ABCA1 transporter regulates cellular cholesterol content and reverses cholesterol transport. The aim of this study was to determine the relationship between single nucleotide polymorphisms (SNPs) R230C, C-17G, and C-69T and their expression in epicardial and mediastinal adipose tissue in Mexican patients with CAD. Subjects and methods: The study included 71 patients with CAD and a control group consisting of 64 patients who underwent heart valve replacement. SNPs were determined using TaqMan probes. mRNA was extracted using TriPure Isolation from epicardial and mediastinal adipose tissue. Quantification and expression analyses were done using RT-qPCR. Results: R230C showed a higher frequency of the GG genotype in the CAD group (70.4%) than the control group (57.8%) [OR 0.34, 95% CI (0.14-0.82) p = 0.014]. Similarly, C-17G (rs2740483) showed a statistically significant difference in the CC genotype in the CAD group (63.3%) in comparison to the controls (28.1%) [OR 4.42, 95% CI (2.13-9.16), p = 0.001]. mRNA expression in SNP R230C showed statistically significant overexpression in the AA genotype compared to the GG genotype in CAD patients [11.01 (4.31-15.24) vs. 3.86 (2.47-12.50), p = 0.015]. Conclusion: The results suggest that the GG genotype of R230C and CC genotype of C-17G are strongly associated with the development of CAD in Mexican patients. In addition, under-expression of mRNA in the GG genotype in R230C is associated with patients undergoing revascularization.

ABSTRACT Objective: To define a reference chart comparing pressure drop vs. flow generated by a set of arterial cannulae currently utilized in cardiopulmonary bypass conditions in pediatric surgery. Methods: Cannulae from two manufacturers were selected considering their design and outer and inner diameters. Cannula performance was evaluated in terms of pressure drop vs. flow during simulated cardiopulmonary bypass conditions. The experimental circuits consisted of a Jostra HL-20 roller pump, a Quadrox-i pediatric oxygenator (Maquet Cardiopulmonary AG, Rastatt, Germany), and a custom pediatric tubing set. The circuit was primed with lactated Ringer's solution only (first condition) and with human packed red blood cells added (second condition) to achieve a hematocrit of 30%. Cannula sizes 8 to 16 Fr were inserted into the cardiopulmonary bypass circuit with a "Y" connector. The flow was adjusted in 100 ml/min increments within typical flow ranges for each cannula. Pre-cannula and post-cannula pressures were measured to calculate the pressure drop. Results: Utilizing a pressure drop limit of 100 mmHg, our results suggest a recommended flow limit of 500, 900, 1400, 2600, and 3100 mL/min for Braile arterial cannulae sizes 8, 10, 12, 14, and 16 Fr, respectively. For Medtronic DLP arterial cannulae sizes 8, 10, 12, 14, and 16 Fr, the recommended flow limit is 600, 1100, 1700, 2700, and 3300 mL/min, respectively. Conclusion: This study reinforces discrepancies in pressure drop between cannulae of the same diameter supplied by different manufacturers and the importance of independent translational research to evaluate components' performance.

ABSTRACT

Antecedentes. El valor pronóstico de una ergometría positiva en el contexto de imágenes tomográficas de perfusión miocárdica de estrés y reposo (SPECT) normales no está bien establecido. Objetivos. Documentar la incidencia de infarto, muerte y revascularización coronaria en pacientes con una ergometría positiva de riesgo intermedio e imágenes de perfusión SPECT normales, y explorar el potencial valor del puntaje de riesgo de Framingham en la estratificación pronóstica de estos pacientes. Métodos. Cohorte retrospectiva integrada por pacientes que habían presentado síntomas o hallazgos electrocardiográficos compatibles con enfermedad arterial coronaria durante la prueba de esfuerzo, con criterios de riesgo intermedio en la puntuación de Duke y perfusión miocárdica SPECT normal. Fueron identificados a partir de la base de datos del laboratorio de cardiología nuclear del Instituto de Cardiología y Cirugía Cardiovascular de la ciudad de Posadas, Argentina. Resultados. Fueron elegibles 217 pacientes. El seguimiento fue de 3 1,5 años. La sobrevida libre de eventos (muerte,infarto de miocardio no fatal, angioplastia coronaria o cirugía de bypass de arteria coronaria) a uno, tres y cinco años fue significativamente menor (Log-rank test, p= 0,001) en el grupo con puntaje de Framingham alto o muy alto (77, 71y 59 %, respectivamente) que en el grupo de puntaje bajo o intermedio (89, 87 y 83 %). Tomando como referencia a los pacientes con riesgo bajo en el puntaje de Framingham, luego de ajustar por edad, sexo y puntaje de Duke, los pacientes categorizados en los estratos alto y muy alto riesgo del puntaje de Framingham presentaron una incidencia del evento combinado cercana al triple (hazard ratio [HR] 2,81; intervalo de confianza [IC] del 95 % 0,91 a 8,72; p= 0,07 y HR 3,61;IC 95 % 1,23 a 10,56; p= 0,019 respectivamente). Conclusiones. La estimación de riesgo con el puntaje de Framingham sería de ayuda en la estratificación pronóstica de los pacientes con ergometría positiva y SPECT normal. (AU)


Background. The prognostic value of positive exercise testing with normal SPECT myocardial perfusion imaging is not well established. Objectives. To document the incidence of infarction, death, and coronary revascularization in patients with a positive intermediate-risk exercise test and normal SPECT perfusion images and to explore the potential value of the Framingham Risk Score in the prognostic stratification of these patients. Methods. A retrospective cohort comprised patients who presented symptoms or electrocardiographic findings compatible with coronary artery disease during the stress test, with intermediate risk criteria in the Duke score and normal SPECT myocardial perfusion. They were identified from the database of the nuclear cardiology laboratory of the Instituto de Cardiología y Cirugía Cardiovascular of Posadas, Argentina. Results. 217 patients were eligible. Follow-up was 3 1.5 years. Event-free survival (death, non-fatal myocardial infarction, coronary angioplasty, or coronary artery bypass surgery) at one, three, and five years was significantly lower (Log-ranktest, p: 0.001) in the group with a score of Framingham high or very high (77, 71 and 59 %, respectively) than in the lowor intermediate score group (89, 87 and 83 %). Taking as reference the low-risk patients in the Framingham score, after adjusting for age, sex, and Duke score, the patients categorized in the high-risk and very high-risk strata showed about three times higher incidence of the combined event (hazard ratio [HR] 2.81; 95 % confidence interval [CI] 0.91 to 8.72;p=0.07 and HR 3.61; 95 % CI 1.23 to 10.56; p=0.019 respectively). Conclusions. Risk estimation with the Framingham score would be helpful in the prognostic stratification of patients with positive exercise testing and normal SPECT. (AU)

ABSTRACT

@#Objective     To evaluate the risk factors for postoperative in-hospital mortality in elderly patients receiving cardiac valvular surgery, and develop a new prediction models using the least absolute shrinkage and selection operator (LASSO)-logistic regression. Methods     The patients≥65 years who underwent cardiac valvular surgery from 2016 to 2018 were collected from the Chinese Cardiac Surgery Registry (CCSR). The patients who received the surgery from January 2016 to June 2018 were allocated to a training set, and the patients who received the surgery from July to December 2018 were allocated to a testing set. The risk factors for postoperative mortality were analyzed and a LASSO-logistic regression prediction model was developed and compared with the EuroSCOREⅡ. Results     A total of 7 163 patients were collected in this study, including 3 939 males and 3 224 females, with a mean age of 69.8±4.5 years. There were 5 774 patients in the training set and 1 389 patients in the testing set. Overall, the in-hospital mortality was 4.0% (290/7 163). The final LASSO-logistic regression model included 7 risk factors: age, preoperative left ventricular ejection fraction, combined coronary artery bypass grafting, creatinine clearance rate, cardiopulmonary bypass time, New York Heart Association cardiac classification. LASSO-logistic regression had a satisfying discrimination and calibration in both training [area under the curve (AUC)=0.785, 0.627] and testing cohorts (AUC=0.739, 0.642), which was superior to EuroSCOREⅡ. Conclusion     The mortality rate for elderly patients undergoing cardiac valvular surgery is relatively high. LASSO-logistic regression model can predict the risk of in-hospital mortality in elderly patients receiving cardiac valvular surgery.

ABSTRACT

Diabetic retinopathy(DR) and coronary heart disease(CHD) are both major chronic vascular complications that seriously jeopardize the health of the population and often occur together in clinical practice, it is of great clinical value to actively explore the association between the two in the process of disease development and methods of prevention and treatment of modern medicine and traditional Chinese medicine(TCM). According to TCM, the heart and eyes physiologically communicate with each other by taking Qi, blood and veins as bridges, blood stasis obstructing collaterals is the common TCM etiology of DR and CHD, whose mechanism involves inflammation, oxidative stress and endothelial dysfunction. Promoting blood circulation and removing blood stasis plays an important role in the same treatment for different diseases and prevention and treatment of comorbidities, possibly by inhibiting the expression of interleukin-1β(IL-1β), endothelin-1(ET-1) and hypoxia inducible factor-1α/vascular endothelial growth factor(HIF-1α/VEGF), regulating phosphatidylinositol 3-kinases/protein kinase B/mammalian target of rapamycin(PI3K/Akt/mTOR) pathway, initiating adenosine monophosphate(AMP)-activated protein kinase/silent information regulator 1(AMPK/SIRT1) and nuclear transcription factor erythroid 2-related factor 2/heme oxygenase-1(Nrf2/HO-1) signaling pathways, inhibiting Hippo/Yes-associated protein(Hippo/YAP) signaling pathway, inhibiting mitochondrial permeability transition pore and anti-platelet agglutination for treating DR and CHD, which provides a multi-component, multi-pathway and multi-target selection strategies and ideas for the prevention and treatment of DR and CHD by TCM from a biological perspective. Based on this, subsequent studies should focus on constructing clinically relevant comorbidity models, conducting multicenter prospective studies, and fully utilizing artificial intelligence technology to gain a deeper understanding of the relationship between the two diseases, so as to elucidate the mechanism of promoting blood circulation and removing blood stasis in preventing and treating panvascular diseases.

Objective To establish an individual Nomgram model for predicting the risk of coronary heart disease complicated with pulmonary hypertension. Methods From January 2017 to December 2021 , 352 patients with coronary heart disease (CHD) complicated with pulmonary hypertension in our hospital were selected, and 352 patients with coronary heart disease but without pulmonary hypertension were selected as the control group. The clinical baseline data of the two groups were analyzed first, and then logistics multivariate analysis was performed. To explore the risk factors of coronary heart disease complicated with pulmonary hypertension, the Nomgram model was established to predict the risk, and the predictive value of the model was tested by receiver characteristic curve (ROC). Results Logistics multivariate analysis showed that alcoholism, smoking, stroke history, hypertension course, CHD course, PASP, HCT, PaCO2, D-dimer, NIHSS score and low PaO2 were all independent risk factors for CHD complicated with pulmonary hypertension. Nomgram model prediction results for patients with coronary heart disease showed that Alcohol abuse, smoking, stroke history, duration of hypertension (5.66 years), duration of coronary heart disease (2.12 years), NIHSS (12.33 points), PASP (75.22mmHg), HCT (33.22%), PaCO2 (56.11mmHg), D-dimer (255.12μg/L), PaO2 (56.22mmHg) is a risk factor for coronary heart disease complicated with pulmonary hypertension. ROC curve showed that the area under the prediction curve of Nomgram model for coronary heart disease complicated with pulmonary hypertension was 0.675. Conclusion Nomgram model can predict pulmonary hypertension in patients with coronary heart disease to a certain extent.

ABSTRACT

The key pathogenesis of coronary heart disease (CHD) is spleen deficiency and phlegm stasis, and dysfunctional high-density lipoprotein (dys-HDL) may be the biological basis for the occurrence of CHD due to spleen deficiency and phlegm stasis. Considering the biological properties and effects of high-density lipoprotein (HDL), it is believed that the structure and components of HDL are abnormal in the state of spleen deficiency which led to dys-HDL; and dys-HDL contributes to the formation of atherosclerotic plaques through two major pathways, namely, mediating the dysfunction of endothelial cells and mediating the foaminess of macrophages and smooth muscle cells, thus triggering the development of CHD. It is also believed that dys-HDL is a microcosmic manifestation and a pathological product of spleen deficiency, and spleen deficiency makes foundation for the production of dys-HDL; dys-HDL is also an important biological basis for the phlegm-stasis interactions in CHD. The method of fortifying spleen, resolving phlegm, and dispelling stasis, is proposed as an important principle in the treatment of CHD by traditional Chinese medicine, which can achieve the therapeutic purpose by affecting the changes in the structure and components of dys-HDL, thus revealing the scientific connotation of this method, and providing ideas for the diagnosis and treatment of CHD by traditional Chinese medicine.

Introducción: la resiliencia es la capacidad de adaptación de los individuos frente a los retos de la vida. Objetivo: determinar el nivel de resiliencia y su relación con factores de riesgo cardiovascular en médicos residentes del Hospital de Clínicas. Metodología: estudio observacional, analítico, transversal, retrospectivo. Muestreo no probabilístico, realizado en el periodo de mayo a julio de 2022. Se utilizó la encuesta de Wagnild & Young para evaluar la resiliencia. Se midieron variables sociodemográficas, niveles de resiliencia y factores de riesgo cardiovascular. Se utilizó el software Epi info para el análisis de los datos. A las variables cualitativas se les calculó la frecuencia absoluta y relativa y a las variables cuantitativas la media y desviación estándar. Para buscar la asociación entre el nivel de la resiliencia y los factores de riesgo cardiovascular se utilizó el Chi cuadrado, se consideró estadísticamente significativo p: <0,05. Resultados: participaron en el estudio 154 residentes, 45,5 % de los residentes presentaron sobrepeso u obesidad, 87,7 % consumen alcohol, 14,9 % sedentarismo, 5,2 % Hipertensión Arterial, 10,4 % dislipidemia y el 50 % antecedentes familiares de enfermedad cardiovascular. El 48 % de los médicos presentó baja resiliencia. Se encontró relación estadísticamente significativa entre la resiliencia baja y factores de riesgo cardiovascular (p <0,05). Conclusión: la mitad de los residentes presentan baja resiliencia y existe relación entre factores de riesgo cardiovascular y la baja resiliencia en los médicos residentes del Hospital de Clínicas.


Introduction: resilience is the ability of individuals to adapt to life's challenges. Objective: determine the level of resilience and its relationship with cardiovascular risk factors in resident doctors at the Hospital de Clínicas. Methodology: this was an observational, analytical, cross-sectional and retrospective study. We used non-probability sampling, from May to July 2022. The Wagnild & Young survey was used to evaluate resilience. Sociodemographic variables, resilience levels and cardiovascular risk factors were measured. Epi info software was used for data analysis. The absolute and relative frequency was calculated for the qualitative variables and the mean and standard deviation for the quantitative variables. To find the association between the level of resilience and cardiovascular risk factors, the Chi square was used; p: <0.05 was considered statistically significant. Results: 154 residents participated in the study, 45.5 % of the residents were overweight or obese, 87.7 % consumed alcohol, 14.9 % had a sedentary lifestyle, 5.2 % had high blood pressure, 10.4 % had dyslipidemia and 50 % had a family history of cardiovascular disease. 48 % of respondents presented low resilience. A statistically significant relationship was found between low resilience and cardiovascular risk factors (p <0.05). Conclusion: half of the residents have low resilience and there is a relationship between cardiovascular risk factors and low resilience in resident doctors at the Hospital de Clínicas.

Introducción: la trombólisis intravenosa revolucionó la terapéutica de los pacientes con accidentes cerebrovasculares isquémicos. Objetivo: determinar las características clínicas y tiempo de inicio de tratamiento trombolítico en pacientes con accidente cerebrovascular isquémico en la Unidad de ICTUS del Hospital de Clínicas. Metodología: estudio retrospectivo, observacional, descriptivo, transversal, muestreo no probabilístico, de pacientes con diagnóstico de accidente cerebrovascular isquémico ingresados en la Unidad de ICTUS del Hospital de Clínicas, desde enero del 2015 hasta junio del 2022. Se midieron variables sociodemográficas, tiempo de inicio de tratamiento trombolítico, NIHSS al ingreso, 24 horas y 5 días, escala ASPCT, glicemia, presión arterial sistólica y diastólica, factores de riesgo de enfermedad cardiaca, transformación hemorrágica. Resultados: 10 % de los pacientes cumplieron criterios de trombólisis, edad media 62 ± 1, masculino 59 %. Promedio desde el ingreso hospitalario hasta el goteo del trombolítico fue 44±2 minutos y desde el inicio del cuadro hasta el goteo del trombolítico 195 ± 5 minutos. Los factores de riesgo cardiovascular más frecuentes fueron Hipertensión Arterial y Diabetes Mellitus, en 5 % de los trombolizados ocurrieron transformaciones hemorrágicas sintomáticas. Conclusión: 10 % de los pacientes cumplieron criterios para trombólisis. La media desde el ingreso al hospital hasta el goteo del fibrinolítico fue de 44 minutos y desde el inicio del cuadro hasta el goteo del trombolítico 195 ± 5 minutos. Los factores de riesgo de enfermedad cardiaca más frecuentes fueron la Hipertensión Arterial y Diabetes Mellitus, ocurrió 5% de transformación hemorrágica sintomática.

Objective: to determine the clinical characteristics and time of initiation of thrombolytic treatment in patients with ischemic stroke in the Stroke Unit of the Hospital de Clínicas. Methodology: this was a retrospective, observational, descriptive, cross-sectional study, we used non-probabilistic sampling, of patients with a diagnosis of ischemic stroke admitted to the Stroke Unit of the Hospital de Clínicas, from January 2015 to June 2022. Sociodemographic variables, start time of thrombolytic treatment, NIHSS at admission, 24 hours and 5 days, ASPCT scale, glycemia, systolic and diastolic blood pressure, risk factors for heart disease, hemorrhagic transformation. Results: 10 % of patients met thrombolysis criteria, with a mean age of 62 ± 1,59 % were male. The average time from hospital admission to the thrombolytic drip was 44 ± 2 minutes and from the onset of symptoms to the thrombolytic drip was 195 ± 5 minutes. The most frequent cardiovascular risk factors were High Blood Pressure and Diabetes Mellitus; symptomatic hemorrhagic transformations occurred in 5 % of the thrombolyzed patients. Conclusion: 10 % of patients met criteria for thrombolysis. The mean time from hospital admission to the fibrinolytic drip was 44 minutes and from the onset of symptoms to the thrombolytic drip was 195 ± 5 minutes. The most frequent risk factors for heart disease were High Blood Pressure and Diabetes Mellitus, 5 % of symptomatic hemorrhagic transformation occurred.

Resumen Antecedentes y objetivo: El infarto prematuro tiene un alto impacto socioeconómico y carecemos de estudios contemporáneos que analicen las características asociadas al desarrollo de estos eventos y a su evolución Material y meéodos: En un estudio observacional sobre una cohorte retrospectiva que incluyó pacientes menores de 46 años con un infarto entre junio de 2012 y junio de 2022, se analizaron las características sociodemográficas, factores de riesgo cardiovasculares, tóxicos, características del infarto, datos sobre infección e inmunización por SARS-CoV-2, se realizó un seguimiento retrospectivo a un anÌo mediante la historia electrónica y se compararon los resultados con los reportados en la literatura. Resultados: El 12.1% de las angioplastias primarias corresponded a infartos prematuros (103). Entre los factores de riesgo clásicos destacaron la dislipidemia y el sobrepeso. La hipertensión y la diabetes fueron significativamente menores que las cifras reportadas en población mayor. El consumo de tabaco fue significativamente mayor, aí como el de otros tóxicos entre los que destacaron el cannabis y la ocaína. Los antecedentes familiares estuvieron presentes en 27 pacientes. El 27.2% (28) tuvieron complicaciones durante el ingreso y la mortalidad fue del 4.9% (5). No se identificaron diferencias significativas entre los pacientes que iniciaron con un evento antes o despuéde la pandemia, a excepción de una tendencia a la disminción del hábito tabáquico. Conclusiones: El infarto prematuro es una patología al alza con una morbimortalidad considerable. La concienciación sobre los hábitos tóxicos y prevención de los factores de riesgo clásicos son tareas prioritarias en nuestro medio.


Abstract Background and purpose: Premature infarction has a high socioeconomic impact and we lack contemporary studies that analyze the characteristics associated with its development and evolution. Material and methods: In an observational study on a retrospective cohort of patients younger than 46 years old, who had suffered from an infarction from June 2012 to June 2022, sociodemographic characteristics, cardiovascular and toxic risk factors, infarction characteristics, SARS-CoV-2 infection and immunization data were investigated. Indeed, a one-year retrospective follow-up was performed through their electronic health record and results were compared with the ones reported in literature. Results: 12.1% of primary angioplasties corresponded to premature infarctions (103). Among classical risk factors, dyslipidemia and overweight stood out, while hypertension and diabetes incidence was significantly lower than the one reported in older population. The consumption of tobacco and other drugs (especially cannabis and cocaine) was higher than that reported at older ages. Twenty-seven patients had a family history of ischemic cardiopathy or sudden cardiac death, 27.2% (28) had complications during admission and mortality was 4.9% (5). No significant differences were identified between the patients who debuted with an event before or after the pandemic, except for a tendency to smoking cessation. Conclusions: Premature infarction is a rising pathology with considerable morbidity and mortality. Raising awareness about toxic habits and prevention of the classical risk factors are priority tasks in this setting.

Resumen Objetivo: Describir los tipos de cateterismo cardiaco que se realizan en el único laboratorio de hemodinamia pediátrica dentro de la red de salud pública de El Salvador, en colaboración con organizaciones sin fines de lucro. Material y método: Se realizó un estudio descriptivo retrospectivo de corte transversal en el periodo entre mayo de 2022 a enero de 2023, revisando los expedientes de todos los pacientes que pasaron a cateterismo cardiaco. Resultados: Se realizaron 54 procedimientos dentro de la sala de hemodinamia, 37 pacientes de sexo femenino. El peso fue de 20.6 ± 14.5 kg, de los procedimientos, 47 (87%) fueron terapéuticos y 7 (13%) diagnósticos. La oclusión del conducto arterioso se realizó en el 46% de los pacientes. No se reportaron complicaciones, los pacientes fueron dados de alta luego de 12 horas, todos los insumos fueron donados por fundaciones sin fines de lucro. Conclusiones: Nuestro laboratorio de hemodinamia realiza procedimientos de complejidad variada sin reportar hasta el momento complicaciones mayores. Nos vemos limitados debido al costo alto de algunos dispositivos, a la baja frecuencia de la utilización de estos y a la imposibilidad para adquirirlos en el mercado nacional.


Abstract Objective: To describe the types of cardiac catheterization that are performed in the only pediatric catheterization laboratory within the public health network of El Salvador, in collaboration with non-profit organizations. Material and method: A descriptive, retrospective, cross-sectional study was conducted in the period from May 2022 to January 2023, reviewing the records of all patients who underwent cardiac catheterization. Results: 54 procedures were performed in the catheterization room, 37 female patients. The weight was 20.6 ± 14.5 kg, of the procedures, 47 (87%) were therapeutic and 7 (13%) diagnostic. Occlusion of the ductus arteriosus was performed in 46% of the patients. No complications were reported, the patients were discharged after 12 hours, all supplies were donated by non-profit foundations. Conclusions: Our catheterization laboratory performs procedures of varied complexity without reporting any major complications to date. We are limited due to the high cost of some devices, the low frequency of their use and the impossibility of acquiring them in the national market.

Resumen Antecedentes: La obesidad es un trastorno multifactorial caracterizado por un aumento en la adiposidad corporal, de amplia prevalencia en nuestro país, a cualquier edad y ligada a grandes consecuencias adversas, incluyendo el desarrollo de cardiopatías. Los programas de rehabilitación cardiaca (RC) son intervenciones interdisciplinarias encaminadas no solo a restaurar la funcionalidad perdida de los pacientes que han sufrido un desenlace cardiovascular, sino también a corregir aquellos factores de riesgo que lo propiciaron y que interfieren en los resultados adaptativos del mismo. La obesidad contribuye a perpetuar el riesgo de cardiopatía y suele ser resistente a las modificaciones del estilo de vida de manera convencional. Objetivo: Establecer pautas en el reconocimiento de la obesidad con directrices de atención al paciente con cardiopatía dentro de los programas de RC y su abordaje interdisciplinario. Método: A través de una extensiva revisión bibliográfica y después de una discusión interdisciplinaria, se elaboró el presente documento para fijar una postura sobre el abordaje de la obesidad en el contexto de los programas de RC en pacientes con cardiopatía. Resultados y conclusiones: Nuestra Sociedad reconoce el abordaje interdisciplinario de los pacientes con obesidad y cardiopatía en su prevención primaria y secundaria, insta a la precisión en su diagnóstico y valoración, y recomienda que su eje primario debe estar basado en primera instancia en las modificaciones del estilo de vida (entrenamiento físico, atención nutricional e intervención psicoemocional), en tanto que la terapia farmacológica y la cirugía bariátrica pudieran ser coadyuvantes en la optimización de los resultados en pacientes selectos.


Abstract Background: Obesity is a multifactorial disorder characterized by increased body adiposity with a wide prevalence in our country, at any age, and linked to major adverse consequences, including the development of heart disease. Cardiac rehabilitation (CR) programs are interdisciplinary interventions aimed not only at restoring the lost functionality of patients who have suffered a cardiovascular outcome, but also at correcting those risk factors that led to it and that interfere with its adaptive results. Obesity contributes to perpetuating heart disease risk and is often resistant to conventional lifestyle modifications. Objective: Establish guidelines in the recognition of obesity with care guidelines for patients with heart disease within CR programs and their interdisciplinary approach. Method: Through an extensive bibliographical review and after an interdisciplinary discussion, this document was prepared to establish a position on the approach to obesity in the context of CR programs in patients with heart disease. Results and conclusions: Our Society recognizes the interdisciplinary approach of our patients with obesity and heart disease in its primary and secondary prevention, urges precision in its diagnosis and assessment, recommends that its primary axis must be based in the first instance on lifestyle modifications (physical training, nutritional care and psycho-emotional intervention), while pharmacological therapy and bariatric surgery could be coadjuvants in optimizing the results in selected patients.

Resumen Introducción: El origen aórtico anómalo de las arterias coronarias tiene una prevalencia estimada del 0.02-5.7% y están potencialmente involucradas con complicaciones en su evolución natural o aquellos que requieran procedimientos intervencionistas hemodinámico y/o quirúrgicos. Puede asociarse a muerte súbita o dañarse durante intervenciones sobre el anillo mitral, aórtico, pulmonar o el cierre percutáneo de un defecto septal interauricular. Objetivo: identificar estos pacientes por diferentes técnicas de imágenes como el ecocardiograma Doppler color transtorácico (ETT), angiotomografía o angiografía coronaria. Métodos: Las técnicas de imágenes utilizadas para la detección de anomalías coronarias fueron el ETT, angiotomografía coronaria multicorte o angiografía coronaria convencional de acuerdo con lo universalmente aceptado. Resultados: Estudio prospectivo realizado desde enero del 2020 a junio del 2021. Se identificaron 15 pacientes y en 12/15 la sospecha fue por ETT y en los tres restantes por angiotomografía coronaria. La arteria circunfleja fue la más involucrada en forma aislada o asociada a otra anomalía coronaria (12/15 pacientes) y en los tres casos restantes la coronaria anómala tuvo un trayecto interarterial, siendo las arterias coronarias derecha y la descendente anterior las afectadas. Conclusiones: El subdiagnóstico por ETT de las anomalías de las arterias coronarias puede deberse a la dificultad para su visualización, sobre todo en la población adulta. Su detección es crucial, ya que puede generar muerte súbita asociada a isquemia miocárdica y arritmias graves o complicar procedimientos intervencionistas sobre el septum interauricular o sobre los anillos mitral, pulmonar y/o aórtico.


Abstract Introduction: The anomalous aortic origin of the coronary arteries has an estimated prevalence of 0.02-5.7%. It can be associated with sudden death when it has an interarterial or intramural pathway or be damaged during interventions on the mitral, pulmonary and/or aortic annulus or percutaneous closure of an interatrial septal defect. Objective: To identify these patients by imaging techniques such as transthoracic color Doppler echocardiography (TTE), computed tomography (CT) multislice angiography or coronary angiography. Methods: The imaging techniques used for the detection of coronary anomalies were TTE, multislice coronary angiography or coronary angiography according to what is generally accepted. Results: Fifteen patients were identified; in 12 of them the suspicion was due to TTE and in the remaining 3, CT multislice angiography was diagnostic. The circumflex artery was the coronary artery most involved, associated or not with another coronary anomaly (12/15 patients) and in the other three cases, the anomalous coronary artery had an interarterial course, with the right coronary arteries and the anterior descending coronary arteries being involved. Conclusions: The under diagnosis by TTE of coronary artery abnormalities may be due to the difficulty of visualization that is accentuated with age. Their detection is crucial because they can both, lead to sudden death associated with an intramural and/or interarterial pathway and complicate an interventional procedure on the interatrial septum or within the mitral, pulmonary and/or aortic rings.

Introducción: la resiliencia es la capacidad de adaptación de los individuos frente a los retos de la vida. Objetivo: determinar el nivel de resiliencia y su relación con factores de riesgo cardiovascular en médicos residentes del Hospital de Clínicas. Metodología: estudio observacional, analítico, transversal, retrospectivo. Muestreo no probabilístico, realizado en el periodo de mayo a julio de 2022. Se utilizó la encuesta de Wagnild & Young para evaluar la resiliencia. Se midieron variables sociodemográficas, niveles de resiliencia y factores de riesgo cardiovascular. Se utilizó el software Epi info para el análisis de los datos. A las variables cualitativas se les calculó la frecuencia absoluta y relativa y a las variables cuantitativas la media y desviación estándar. Para buscar la asociación entre el nivel de la resiliencia y los factores de riesgo cardiovascular se utilizó el Chi cuadrado, se consideró estadísticamente significativo p: <0,05. Resultados: participaron en el estudio 154 residentes, 45,5 % de los residentes presentaron sobrepeso u obesidad, 87,7 % consumen alcohol, 14,9 % sedentarismo, 5,2 % Hipertensión Arterial, 10,4 % dislipidemia y el 50 % antecedentes familiares de enfermedad cardiovascular. El 48 % de los médicos presentó baja resiliencia. Se encontró relación estadísticamente significativa entre la resiliencia baja y factores de riesgo cardiovascular (p <0,05). Conclusión: la mitad de los residentes presentan baja resiliencia y existe relación entre factores de riesgo cardiovascular y la baja resiliencia en los médicos residentes del Hospital de Clínicas.


Introduction: resilience is the ability of individuals to adapt to life's challenges. Objective: determine the level of resilience and its relationship with cardiovascular risk factors in resident doctors at the Hospital de Clínicas. Methodology: this was an observational, analytical, cross-sectional and retrospective study. We used non-probability sampling, from May to July 2022. The Wagnild & Young survey was used to evaluate resilience. Sociodemographic variables, resilience levels and cardiovascular risk factors were measured. Epi info software was used for data analysis. The absolute and relative frequency was calculated for the qualitative variables and the mean and standard deviation for the quantitative variables. To find the association between the level of resilience and cardiovascular risk factors, the Chi square was used; p: <0.05 was considered statistically significant. Results: 154 residents participated in the study, 45.5 % of the residents were overweight or obese, 87.7 % consumed alcohol, 14.9 % had a sedentary lifestyle, 5.2 % had high blood pressure, 10.4 % had dyslipidemia and 50 % had a family history of cardiovascular disease. 48 % of respondents presented low resilience. A statistically significant relationship was found between low resilience and cardiovascular risk factors (p <0.05). Conclusion: half of the residents have low resilience and there is a relationship between cardiovascular risk factors and low resilience in resident doctors at the Hospital de Clínicas.

ABSTRACT

Introducción: la trombólisis intravenosa revolucionó la terapéutica de los pacientes con accidentes cerebrovasculares isquémicos. Objetivo: determinar las características clínicas y tiempo de inicio de tratamiento trombolítico en pacientes con accidente cerebrovascular isquémico en la Unidad de ICTUS del Hospital de Clínicas. Metodología: estudio retrospectivo, observacional, descriptivo, transversal, muestreo no probabilístico, de pacientes con diagnóstico de accidente cerebrovascular isquémico ingresados en la Unidad de ICTUS del Hospital de Clínicas, desde enero del 2015 hasta junio del 2022. Se midieron variables sociodemográficas, tiempo de inicio de tratamiento trombolítico, NIHSS al ingreso, 24 horas y 5 días, escala ASPCT, glicemia, presión arterial sistólica y diastólica, factores de riesgo de enfermedad cardiaca, transformación hemorrágica. Resultados: 10 % de los pacientes cumplieron criterios de trombólisis, edad media 62 ± 1, masculino 59 %. Promedio desde el ingreso hospitalario hasta el goteo del trombolítico fue 44±2 minutos y desde el inicio del cuadro hasta el goteo del trombolítico 195 ± 5 minutos. Los factores de riesgo cardiovascular más frecuentes fueron Hipertensión Arterial y Diabetes Mellitus, en 5 % de los trombolizados ocurrieron transformaciones hemorrágicas sintomáticas. Conclusión: 10 % de los pacientes cumplieron criterios para trombólisis. La media desde el ingreso al hospital hasta el goteo del fibrinolítico fue de 44 minutos y desde el inicio del cuadro hasta el goteo del trombolítico 195 ± 5 minutos. Los factores de riesgo de enfermedad cardiaca más frecuentes fueron la Hipertensión Arterial y Diabetes Mellitus, ocurrió 5% de transformación hemorrágica sintomática.


Introduction: intravenous thrombolysis revolutionized the therapy of patients with ischemic strokes. Objective: to determine the clinical characteristics and time of initiation of thrombolytic treatment in patients with ischemic stroke in the Stroke Unit of the Hospital de Clínicas. Methodology: this was a retrospective, observational, descriptive, cross-sectional study, we used non-probabilistic sampling, of patients with a diagnosis of ischemic stroke admitted to the Stroke Unit of the Hospital de Clínicas, from January 2015 to June 2022. Sociodemographic variables, start time of thrombolytic treatment, NIHSS at admission, 24 hours and 5 days, ASPCT scale, glycemia, systolic and diastolic blood pressure, risk factors for heart disease, hemorrhagic transformation. Results: 10 % of patients met thrombolysis criteria, with a mean age of 62 ± 1,59 % were male. The average time from hospital admission to the thrombolytic drip was 44 ± 2 minutes and from the onset of symptoms to the thrombolytic drip was 195 ± 5 minutes. The most frequent cardiovascular risk factors were High Blood Pressure and Diabetes Mellitus; symptomatic hemorrhagic transformations occurred in 5 % of the thrombolyzed patients. Conclusion: 10 % of patients met criteria for thrombolysis. The mean time from hospital admission to the fibrinolytic drip was 44 minutes and from the onset of symptoms to the thrombolytic drip was 195 ± 5 minutes. The most frequent risk factors for heart disease were High Blood Pressure and Diabetes Mellitus, 5 % of symptomatic hemorrhagic transformation occurred.

Las enfermedades cardiovasculares (Cv), son las causantes de la mayor parte de fallecimientos, como consecuencia de dislipidemia y enfermedad renal crónica (ERC). En Ecuador uno de cada cinco individuos padece de hipertensión arterial, patología que está directamente relacionada con las enfermedades cardiovasculares y la enfermedad renal crónica. Objetivo. Caracterizar el perfil de riesgo cardiovascular en pacientes con enfermedad renal crónica y su asociación con la presencia de dislipidemia, atendidos en el Hospital Homero Castanier de la ciudad de Azogues, durante el periodo de enero a diciembre de 2021. Materiales y Métodos. Se llevó a cabo una investigación de diseño observacional, descriptivo y retrospectivo donde se analizaron 104 historias clínicas de pacientes, correspondientes al periodo comprendido entre enero y diciembre de 2021. Para el cálculo del riesgo cardiovascular se empleó la tabla de predicción del riesgo AMR-D de la Organización Mundial de la Salud/Sociedad Internacional de Hipertensión para el continente americano categoría D. Los datos fueron analizados en el programa SPSS, mediante estadística descriptiva e inferencial. Resultados. De los datos analizados de los 104 pacientes con enfermedad renal crónica el 44,2% presentaron dislipidemia, al mismo tiempo el 74% de los pacientes en estudio manifestaron un riesgo cardiovascular bajo, seguido del 13,5% con riesgo moderado; destacando entre las principales comorbilidades la hipertensión arterial y la diabetes mellitus. Conclusiones. Se caracterizó el riesgo cardiovascular de los pacientes con ERC atendidos en el Hospital Homero Castanier de la ciudad de Azogues en el periodo enero - diciembre 2021, encontrando una baja prevalencia de desarrollar riesgo cardiovascular.


Cardiovascular diseases (CVD) are responsible for most deaths as a consequence of dyslipidemia and chronic kidney disease (CKD). In Ecuador, one out of every five individuals suffers from arterial hypertension, a pathology that is directly related to cardiovascular diseases and chronic kidney disease. Objective. To characterize the cardiovascular risk profile in patients with chronic kidney disease and its association with the presence of dyslipidemia, attended at the Homero Castanier Hospital in the city of Azogues, during the period from January to December 2021. Materials and Methods. An observational, descriptive and retrospective research design was carried out in which 104 clinical histories of patients were analyzed, corresponding to the period between January and December 2021. The AMR-D risk prediction table of the World Health Organization/International Society of Hypertension for the Americas category D was used to calculate cardiovascular risk. The data were analyzed in the SPSS program, using descriptive and inferential statistics. Results. Of the data analyzed, 44.2% of the 104 patients with chronic kidney disease presented dyslipidemia; at the same time, 74% of the patients in the study showed low cardiovascular risk, followed by 13.5% with moderate risk, with arterial hypertension and diabetes mellitus standing out among the main comorbidities. Conclusions. The cardiovascular risk of patients with CKD treated at the Homero Castanier Hospital in the city of Azogues during the period January-December 2021 was characterized, finding a low prevalence of developing cardiovascular risk.


As doenças cardiovasculares (DCV) são responsáveis pela maioria das mortes como consequência da dislipidemia e da doença renal crônica (DRC). No Equador, um em cada cinco indivíduos sofre de hipertensão arterial, uma patologia que está diretamente relacionada às doenças cardiovasculares e à doença renal crônica. Objetivo. Caracterizar o perfil de risco cardiovascular em pacientes com doença renal crônica e sua associação com a presença de dislipidemia, atendidos no Hospital Homero Castanier, na cidade de Azogues, durante o período de janeiro a dezembro de 2021. Materiais e métodos. Foi realizado um desenho de pesquisa observacional, descritivo e retrospectivo, no qual foram analisados 104 históricos clínicos de pacientes, correspondentes ao período entre janeiro e dezembro de 2021. A tabela de previsão de risco AMR-D da Organização Mundial da Saúde/Sociedade Internacional de Hipertensão para as Américas categoria D foi usada para calcular o risco cardiovascular. Os dados foram analisados no SPSS usando estatísticas descritivas e inferenciais. Resultados. Dos 104 pacientes com doença renal crônica, 44,2% apresentavam dislipidemia, enquanto 74% dos pacientes em estudo tinham baixo risco cardiovascular, seguidos por 13,5% com risco moderado; as principais comorbidades foram hipertensão e diabetes mellitus. Conclusões. Foi caracterizado o risco cardiovascular dos pacientes com DRC atendidos no Hospital Homero Castanier da cidade de Azogues no período de janeiro a dezembro de 2021, encontrando uma baixa prevalência de desenvolvimento de risco cardiovascular.

Introducción: Uno de los aspectos que más interés suscita en lo referente a la distribución geográfica de la mortalidad por cáncer es la formación de conglomerados espaciales. Objetivo: Identificar el patrón espacial de la mortalidad por cáncer, cardiopatía isquémica y enfermedad cerebrovascular isquémica mediante la detección y descripción de conglomerados espaciales en la provincia de Santiago de Cuba. Métodos: Se realizó un estudio ecológico exploratorio espacial de los fallecimientos por cáncer, cardiopatía isquémica y enfermedad cerebrovascular isquémica durante el período comprendido desde el 1 de enero hasta el 31 de diciembre de 2019 en la provincia de Santiago de Cuba, para lo cual se procedió a la detección de conglomerados espaciales de elevada y baja mortalidad por las causas antes citadas. Se empleó el método de escaneo espacial estadístico con el programa Satscan y las variables independientes fueron divididas en demográficas y clínicas. Resultados: De los 36 conglomerados espaciales de mortalidad detectados, 23 correspondieron al cáncer (65,8 %), 7 a cardiopatía isquémica (18,4 %) y 6 a enfermedad cerebrovascular isquémica (15,8 %); asimismo, 44,7 % del total eran de elevado riesgo y 55,3 % de bajo riesgo. Conclusiones: El patrón de la distribución espacial de mortalidad por cáncer de próstata, pulmón, mama, colon, esófago, cardiopatía isquémica y enfermedad cerebrovascular isquémica se caracterizó por la formación de conglomerados espaciales de elevada y baja mortalidad.


Introduction: One of the aspects that more interest raises regarding the geographical distribution of mortality due to cancer is the formation of space conglomerates. Objective: To identify the space pattern of mortality due to cancer, ischemic heart disease and ischemic cerebrovascular disease by means of the detection and description of space conglomerates in Santiago de Cuba province. Methods: A space exploratory ecological study of deaths due to cancer, ischemic heart disease and ischemic cerebrovascular disease was carried out during January 1st to December 31, 2019 in Santiago de Cuba province, for which space conglomerates of high and low mortality due to the abovementioned causes were detected. The statistical space escanning method was used with the Satscan program and the independent variables were divided in demographic and clinical. Results: Of the 36 space conglomerates of mortality detected, twenty three corresponded to cancer (65.8%), seven to ischemic heart disease (18.4%) and six to ischemic cerebrovascular disease (15.8%); also, 44.7% of the total was of high risk and 55.3% of low risk. Conclusions: The space distribution pattern of mortality due to prostate, lung, breast, colon, esophagus cancer, ischemic heart disease and ischemic cerebrovascular disease was characterized by the formation of high and low mortality space conglomerates.

Objective: Review the relationship between Multiple Sclerosis (MS) and the cardiovascular (CV) system, as well as the CV manifestations of the disease and the CV complications of treatment. Methods: We performed a non-systematic review of the main databases, with no time limit. Results: People with MS tend to have a different CV risk than the general population, with a higher prevalence of hypertension, hyperlipidemia, overweight, ischemic heart disease, and peripheral and cerebral artery disease. In addition, cardiac alterations can be present in any part of MS patient care. Furthermore, MS treatments are not innocuous for the CV system and require attention, especially considering fingolimod and mitoxantrone. Discussion: The findings could partially explain the higher mortality rates found in this population. Furthermore, at the onset, dysautonomia symptoms, like postural orthostatic tachycardia syndrome, can be used as a clinical marker of patients at higher risk to evolve from clinically isolated syndrome to MS. Finally, MS not only progress badly when associated with CV risk factors but are also at increased risk of CV morbidity and mortality. Conclusion: Physicians addressing MS patients should be aware of their increased cardiovascular risk and the impact that adequate control of these factors can have on disease progression, patient lifespan, and global care.


Objetivo: Analisar a relação entre a esclerose múltipla (EM) e o sistema cardiovascular (CV), bem como as manifestações CV da doença e as complicações CV do tratamento. Métodos: Foi realizada uma revisão não sistemática das principais bases de dados, sem limite de tempo. Resultados: Pessoas com EM tendem a ter um risco CV diferente da população em geral, com maior prevalência de hipertensão, hiperlipidemia, sobrepeso, cardiopatia isquêmica e doença arterial periférica e cerebral. Além disso, as alterações cardíacas podem estar presentes em qualquer parte do tratamento do paciente com EM. Além disso, os tratamentos da EM não são inócuos para o sistema CV e requerem atenção, especialmente considerando o fingolimod e a mitoxantrona. Discussão: Os achados podem explicar parcialmente as taxas de mortalidade mais altas encontradas nessa população. Além disso, no início, os sintomas de disautonomia, como a síndrome de taquicardia postural ortostática, podem ser usados como um marcador clínico de pacientes com maior risco de evoluir da síndrome clinicamente isolada para a EM. Por fim, a EM não só progride mal quando associada a fatores de risco CV, mas também apresenta um risco maior de morbidade e mortalidade CV. Conclusão: Os médicos que lidam com pacientes com EM devem estar cientes de seu risco cardiovascular aumentado e do impacto que um controle adequado desses fatores pode ter na progressão da doença, no tempo de vida do paciente e nos cuidados globais.

Resumen Antecedentes: El síndrome de Down es la anomalía cromosómica más frecuente y se asocia con defectos cardiacos congénitos, elementos clínicos de una alta morbilidad y mortalidad infantil. Objetivo: Describir los desenlaces clínicos de los pacientes con síndrome de Down sometidos a cirugía e intervencionismo como tratamiento de las cardiopatías congénitas en esta institución. Material y métodos: Estudio retrospectivo que incluyó pacientes menores de 18 años con síndrome de Down y patología cardiaca asociada durante los últimos 10 años. Las variables estudiadas fueron: peso, talla, sexo, edad, tipo de cardiopatía, procedimiento correctivo, tiempo de estancia intrahospitalaria y en unidad de terapia intensiva, morbilidad y mortalidad. Resultados: 368 pacientes pediátricos fueron llevados a corrección quirúrgica o intervencionista, de los cuales 197 (54%) pertenecían al sexo femenino, la mediana de edad fue de 24 meses (rango intercuartílico [RIQ]: 14-48) en el grupo quirúrgico y de 36 meses (RIQ: 17-85) en el intervencionista. Las cardiopatías congénitas más frecuentes fueron: persistencia del conducto arterioso (31%), comunicación interventricular (28%), canal atrioventricular (CAV) (20%), comunicación interauricular (16%) y tetralogía de Fallot con el 4% respectivamente. La estancia hospitalaria fue de 9 días (RIQ: 7-15) en el grupo quirúrgico y de 3 días (RIQ: 2-5) en el hemodinámico. Las morbilidades fueron infección postoperatoria en 30 pacientes (14%) y en 19 pacientes (9%) bloqueo atrioventricular completo. La mortalidad global incluyendo tanto el quirúrgico como el intervencionista fue del 2%. Conclusiones: Los resultados terapéuticos, quirúrgicos e intervencionistas, en los niños con síndrome de Down y cardiopatías congénitas han mejorado en forma muy satisfactoria. Es de destacar la menor prevalencia del CAV en la población mexicana. Es indispensable realizar evaluación cardiológica a los niños con síndrome de Down y aquellos con cardiopatías congénitas llevarlos a corrección de manera oportuna para favorecer la sobrevida y calidad de vida.


Abstract Background: Down syndrome is the most common chromosomal abnormality, it is associated with a wide variety of congenital heart defects, being considered as clinical elements of high infant morbidity and mortality. Objective: To describe the clinical outcomes of patients with Down syndrome undergoing surgery and interventionism as treatment for congenital heart disease at this Institution. Material and methods: 368 patients with Down syndrome and associated congenital heart disease were diagnosed. The variables studied were weight, stature, sex, age, type of heart disease, corrective procedure, length of stay in the hospital and intensive care unit, morbidity and mortality. Results: 368 pediatric patients underwent surgical or interventional correction. Of which 197 (54%) were female, the median age was 24 months (interquartile range [IQR]: 14-48) in the surgical group and 36 months (IQR: 17-85) in the interventional group. The most frequent congenital heart diseases were: PCA (31%), IVC (28%), CAV (20%), ASD (16%) and tetralogy of Fallot with 4% respectively. Hospital stay was 9 days (IQR: 7-15) in the surgical group and 3 days (IQR: 2-5) in the hemodynamic group. Morbidities were postoperative infection in 30 patients (14%) and complete atrioventricular block in 19 patients (9%). Overall mortality including both surgical and interventional was 2%. Conclusions: The therapeutic, surgical and interventional results in children with Down syndrome and congenital heart disease have improved very satisfactorily. The lower prevalence of the atrioventricular canal in the Mexican population is noteworthy. It is essential to carry out a cardiological evaluation of children with Down syndrome and those with congenital heart disease to correct them in a timely manner to promote survival and quality of life.

Abstract Objective: Associating comorbidities and cardiac symptoms that alter myocardial mechanical function could help clinicians to correctly identify at-risk population. Methods: We conducted a functional open population cross-sectional study of patients referred to a positron emission tomography/computed tomography unit in Mexico City for evaluation of myocardial function, perfusion, and coronary circulation. Ischemia was defined as a sum difference score ≥ 2. Association between comorbidities and cardiac symptoms was tested using logistic regression models and trend analysis. We performed an interaction analysis to evaluate the addition of any accompanying symptoms to comorbid conditions on impairment of myocardial function. Results: One thousand two hundred and seventy-three patients were enrolled, 66.1% male, with a mean age of 62.4 (± 12.7) years, 360 (28.7%) with ischemia, 925 (72.7%) with at least one comorbidity, and 676 (53.1%) had at least one associated cardiac symptom. Patients without ischemia, type 2 diabetes, arterial hypertension, and adverse cardiac symptoms were associated with adverse mechanical, perfusion, and coronary flow parameters. We observed a trend of a cumulative number of comorbidities and cardiac symptoms with increased ischemia and decreased coronary flow. Only in decreased left ventricular ejection fraction, we demonstrated an interaction effect between increased comorbidities and adverse symptoms. Conclusion: The high burden of comorbidities and symptoms in our population alters myocardial function regardless of the level of ischemia.


Resumen Objetivo: La asociación de comorbilidades y síntomas cardíacos que alteran la función miocárdica podría ayudar a los médicos a identificar correctamente a poblaciones de riesgo. Métodos: Se realizó un estudio transversal en población abierta de pacientes referidos a una unidad de PET/CT en la Ciudad de México para evaluación de la función miocárdica, perfusión y circulación coronaria. La isquemia se definió como una suma de diferencia de puntuación (SDS) ≥ 2. La asociación entre las comorbilidades y los síntomas cardíacos se fundamentó mediante modelos de regresión logística y análisis de tendencias. Realizamos un análisis de interacción para evaluar la adición de cualquier síntoma acompañante a condiciones comórbidas en el deterioro de la función miocárdica. Resultados: Se incluyeron 1.273 pacientes, 66,1% del sexo masculino, con una edad media de 62,4 (± 12.7) años, 360 (28,7%) con isquemia, 925 (72,7%) con al menos una comorbilidad y 676 (53,1%) con al menos una menos un síntoma cardíaco asociado. En pacientes sin isquemia, la diabetes mellitus tipo 2, la hipertensión arterial y los síntomas cardíacos adversos se asociaron con parámetros mecánicos, de perfusión y de flujo coronario adversos. Se observó una tendencia con el número acumulado de comorbilidades y síntomas cardíacos con aumento de la isquemia y disminución del flujo coronario. Solo en la disminución de la FEVI se demostró un efecto de interacción entre el aumento de las comorbilidades y los síntomas adversos. Conclusión: La alta carga de comorbilidades y síntomas en nuestra población altera la función miocárdica independientemente del nivel de isquemia.
